Surveys are due by September 1, 2026 so that the District may complete its service-area-wide assessment within the timeframe required by the State. Every customer must complete the survey applicable to their property and return it to the District.

4.  If a backflow prevention assembly is installed on the property, also submit a copy of the most recent test report.
